What to include in a notice

Under 17 U.S.C. § 512(c)(3), a notice must include:

1. Your physical or electronic signature as the copyright owner or a person authorized to act on the owner's behalf.
2. The copyrighted work you claim is infringed, or a representative list if the notice covers multiple works on one site.
3. The material you claim is infringing, with enough information for us to locate it. Include the relevant URL or, for a CCK order, its order number and kiosk location.
4. Your mailing address, telephone number and email address.
5. A statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent or the law.
6. A statement that the information is accurate and, under penalty of perjury, that you are authorized to act on behalf of the owner of the exclusive right allegedly infringed.
